From Vision to Impact
Celebrating 5 Years of Collaborative System Transformation
Dear valued partners, colleagues and friends,
This year marks a milestone in the journey of Stepped Care Solutions: five years of advancing our vision of Wellbeing Everywhere. From our beginnings as a small group that had seen first-hand the transformative impact of matching services to need in accessible, timely, and simple ways, to our current role as a trusted partner in system design across the country and internationally, these five years have shown what is possible when collaboration, innovation, and shared purpose guide our work.
In 2025, our impact deepened across the country and beyond. We supported system transformation work across Canada, and partnered directly with four provincial and territorial governments, advanced collaborations with post-secondary institutions in several countries, and worked with numerous community organizations and healthcare networks to improve access to care. Internationally, our collaborations in Chile and Wales reflect the growing recognition that the flexibility and adaptability of the Stepped Care 2.0® model can meet the universal demand for accessible, person-centred, and recovery-focused systems of support.
As we reflect on this five-year journey, certain milestones stand out: the launch of Wellness Together Canada in 2020 at a moment of national need; the growth of our partnerships with provincial and territorial governments to embed Stepped Care 2.0® into their strategies; the inaugural Better Together Summit in 2025, where 300 leaders and innovators came together to accelerate change; and the launch of our refreshed brand identity, which better reflects who we are today: flexible, accessible, and collaborative. These are not simply organizational achievements—they are proof points of what can be accomplished when diverse voices come together to reimagine how people access mental health resources and support.
Looking ahead, we know our transformation work is ongoing. System change is never static; it evolves with communities, cultures, and needs. Our commitment is to continue scaling solutions through partnership, strengthening what is working well, and ensuring equity remains at the heart of every system. We will keep listening, co-designing, and adapting so that every person can access the right care, at the right time, in the right place.
This report is both a celebration of what we have achieved together and an invitation to join us on our continued journey to transform mental health and substance use health systems for better access and outcomes. The last five years have shown us the power of building together. 2025 Impact at a Glance
Where We Worked
- Worked with clients in nine provinces and territories across Canada, including direct partnerships with four governments (NL, NB, PEI, and NWT).
- Partnered with 13 post-secondary institutions in Canada, the United States, and Chile
- Engaged in research partnerships, including with with McMaster University in Canada and University of California, Berkeley in the U.S.
- Worked with several community-based organizations and healthcare networks.
Training and Capacity Building
- Delivered 20 facilitator-led Single Session Therapy (One-At-A-Time, OAAT) training courses to 275 participants, providing rapid, person-centred and solution-focused support. 88% of participants reported the training was useful and applicable to their work.
- Expanded OAAT training program to offer training in 3 languages (English, French and Spanish) and to include additional offerings:
- Training for clinical supervisors and managers working in an OAAT setting,
- Topic-specific OAAT Focused Conversations, that deepen OAAT therapy skills and practice to engage with all clients in meaningful ways.
- Launched a new eLearning course series focused on Data-Informed Decision making (DIDM) , which is SCS’s approach to measurement based care for all levels of the system. DIDM supports people accessing services, service providers and system administrators to use routine data collection to make decisions about care and service delivery, ensuring built in continuous improvement for positive impacts.
Access and Service Improvements
- Reduced wait times: Partners report significant decreases, with some locations eliminating waitlists altogether.
- Faster, more efficient access: Universities and community sites introduced same-day and next-day appointments, helping people connect with care sooner. Importantly, many had their needs met in a single session easing demand on providers while improving client experience.
- Better outcomes and experiences: Across jurisdictions, partners reported increased client satisfaction, fewer crises, reduced hospitalizations, and a greater sense of safety and inclusion.
Sector Influence and Knowledge Sharing
- Co-authored 5 research and expert commentary pieces in peer-reviewed journals and public forums.
- Better Together Summit: Co-led a national mental health and substance use health event with the MHCC, engaging with 300+ diverse stakeholders and sharing innovative practices.
- Presented at 16 conferences and events, including the Global Leadership Exchange, eMental Health International Collaborative, Institute for Healthcare Improvement Forum, Canadian Health Workforce Conference, and Canada’s Social Prescribing Conference.
- Produced a range of content to broaden public understanding of mental health systems and demonstrate the impact of SC2.0. Our web content, blog articles, podcast interviews and social media channels reached an average of 4,000 people every month.
Five Years of Stepped Care Solutions: Key Milestones
Incorporated in 2020, Stepped Care Solutions celebrates five years as a trusted partner in mental health and substance use health system transformation across Canada and beyond. Our journey builds on strong foundations — from our first research grant and early work in provincial and territorial system change to the launch of Wellness Together Canada and our continued national and international expansion. We’ve built lasting collaborations that are reshaping how people access care. This timeline captures a snapshot of the pivotal moments that have brought our vision to life from 2017 to 2025, as we work toward wellbeing everywhere.
2017
The first provincial implementation of Stepped Care 2.0® (SC2.0) takes place in Newfoundland and Labrador, as part of an e-mental health demonstration project in collaboration with the Mental Health Commission of Canada (MHCC), Memorial University, the Government of Newfoundland and Labrador and Lifewise Mental Health Peer Services (formerly CHANNAL), a lived experience-led peer support organization.
2018
Our work transforming campus mental health and wellness begins at The George Washington University, McGill University in Montreal, and Algonquin College in Ottawa.
2019
A $1.2M CIHR grant funds SC2.0 evaluation across 30 sites in Canada’s four eastern provinces. New Brunswick is the next to join Newfoundland and Labrador in system-level transformation.
2020
Stepped Care Solutions is officially incorporated.
- Wellness Together Canada, the country’s first 24/7 bilingual national portal for mental health and substance use health (MHSUH) resources and services is launched as part of federal government pandemic initiatives.
- The Northwest Territories and Nova Scotia begin transforming mental health systems using SC2.0.
- Developed the Stepped Care 2.0® (SC2.0) Implementation Guide to support consistent, high-quality transformation and scale SC2.0 in more mental health systems and settings. A collaboration with the Mental Health Commission of Canada (MHCC) and the Centre for Addiction and Mental Health (CAMH), the guide offers SC2.0 practice recommendations and an evidence-informed implementation process.
- Stepped Care 2.0: A Paradigm Shift in Mental Health is published, the first book in a series from SCS to support decision-makers and care providers in reimagining delivery of mental health services with an open access model rooted in recovery principles and practice.
2021
- Along with our campus mental health work taking us to University of Toronto, we further expand internationally.
- An ongoing partnership with the University of California, Berkeley begins to transform the school’s service delivery as well as innovate and evaluate digital mental health solutions.
- International collaboration takes us to Chile, with Spanish translations of our One-at-a-Time (OAAT) training.
2022
- Partnered with Foundry BC to support the continued development of their stepped care model to enable youth across British Columbia to easily access dedicated services across a continuum of care across diverse communities.
- Prince Edward Island is the newest province to begin transforming its mental health and substance use health systems with SC2.0.
- Partnered with the Government of Manitoba, organizations, individuals and rights-holders across the province to provide recommendations to build a full continuum of accessible and person-centric services.
2023
- Our work in Ontario expands quickly through collaborations with select Canadian Mental Health Association (CMHA) branches, Ontario Health Teams (OHTs) and post-secondaries, including Cambrian, Humber and Seneca Colleges, to explore and implement stepped care approaches tailored to local contexts.
- Stepped Care 2.0: The Power of Conundrums, the second book in our series, dives deep into the practical application of the SC2.0 model.
- SCS is recognized by The Canadian Alliance on Mental Illness and Mental Health Innovation (CAMIMH) with its Innovation Award.
- We return to Chile, collaborating with the Pontificia Universidad Católica de Chile to address high demand for care and support with a One-at-a-Time (OAAT) counselling approach.
2024
- We supported The Centre of Expertise in Information Technology for Mental Health, Addition and Homelessness (CETI) in mapping the mental health and substance use health systems of Quebec in order to better understand services available and ways to integrate digital mental health solutions province-wide.
- Work is underway to leverage PeerOnCall–a digital peer support application acquired from McMaster University–to support a broader range of workforces with timely, relatable peer connections.
2025
- We partnered with NHS Wales to co-design an implementation roadmap for the country’s new mental health and wellbeing strategy, engaging over 175 stakeholders in person-centered, recovery-focused system planning.
- Work is completed on the third book in our SC2.0 series, Synergy: The Healing Energy Guiding Stepped Care 2.0, and concludes the series when published by Springer in spring 2025. Building on the previous volumes in the series, the new book examines Synergy, the philosophical idea at the heart of SC2.0 and a key for building equitable systems of mental health care.

Across Canada and internationally, our impact has been shaped by collaboration with governments, organizations, post-secondary institutions, communities, and people with lived experience. Strategic Priority 1: Living Our Principles
As part of our commitment to Living Our Principles, we regularly invite feedback from our partners. This process helps us understand how our way of working is experienced and where we can make improvements.
In 2025, partners told us that they value SCS’s collaborative, respectful, and inclusive approach. Through our evaluation process, results highlighted the importance of co-design, safe spaces for participation, timely support, and our focus on community strengths. This feedback affirms our approach and guides us as we continue to adapt and grow.
While feedback has been overwhelmingly positive, we recognise that there is always more to learn, and we will continue to refine our evaluation process to ensure that we capture the full range of perspectives.

Strategic Priority 2: Expanding Reach and Impact
NHS Wales: Building the Foundation for Open Access Mental Health Support
In 2025, NHS Wales launched a bold new mental health and wellbeing strategy focused on prevention, early access, and person-centred care. To help bring this vision to life, Stepped Care Solutions partnered with Welsh system leaders to lay the groundwork for adopting the Stepped Care 2.0 model across all seven health boards.
